Tragus Piercing Guide: Pain, Healing Time and Jewelry

A tragus piercing goes through the tragus, the small flap of cartilage that sits in front of your ear canal. The piercing is quick, usually described as firm pressure and a sharp pinch, and it heals slowly: Cleveland Clinic lists the tragus among cartilage piercings that take 6 to 12 months to heal fully.

This guide explains how the tragus differs from the anti tragus and surface tragus, what the pain feels like, how to heal it around earbuds and phone calls, what the evidence says about migraines, and which jewelry to wear. For every placement on one diagram, see our simple ear map of piercing types.

This article is for informational purposes only and is not medical advice. Get pierced by a licensed professional piercer, and see a doctor for any sign of infection or concern about your piercing.

What Is a Tragus Piercing?

A tragus piercing is a cartilage piercing through the small, firm projection that partly covers the opening of the ear canal. The NIH StatPearls anatomy chapter describes the tragus as a small, triangular cartilaginous projection in front of the ear canal.

Quick fact Tragus piercing
Location Small cartilage flap in front of the ear canal
Tissue Cartilage covered by skin
Typical full healing time 6 to 12 months
Recommended method Hollow needle by a professional piercer
Common starter jewelry Flat back labret stud
Common healed styles Flat back studs, small hoops, huggies
Daily life factor Earbuds and phone calls press on it

Anatomy details come from the NIH StatPearls chapter on the external ear, and healing time from Cleveland Clinic's ear piercing guide.

Tragus vs Anti Tragus vs Surface Tragus

Tragus, anti tragus and surface tragus are three different piercings that share a name. Surface tragus (about 14,800 monthly US searches) and anti tragus (about 12,100) are the most searched variations (DataForSEO, 2026).

Piercing Where it sits Tissue Usual starter jewelry Good to know
Tragus Flap in front of the ear canal Cartilage Flat back labret stud Most common of the three
Anti tragus Ridge above the earlobe, opposite the tragus Cartilage Flat back labret or curved barbell, per piercer Needs enough ridge; not every ear fits
Surface tragus Flat skin on the cheek side, just in front of the tragus Skin surface, not cartilage Surface bar chosen by the piercer Technically a surface piercing with its own jewelry needs

StatPearls places the anti tragus above the lobule and below the helix, lateral to the tragus. Ask your piercer to check your anatomy first, because a small or flat tragus may not hold jewelry well.

Does a Tragus Piercing Hurt?

A tragus piercing usually hurts more than an earlobe piercing because the needle passes through cartilage, not soft fatty tissue. Cleveland Clinic family physician Shannon Thompson, DO, explains that earlobes have few nerves and hurt the least, while cartilage is thicker with more nerves.

Tragus pain has a few specific traits. The tragus is small and firm, so the piercer supports it from inside the ear, which many people feel as pressure before the pinch. Because the tragus sits right at the ear canal, people often say the sound of the needle is more noticeable than the pain itself. The sharp part lasts seconds, and a dull ache can follow for a few days.

No clinical pain scale exists for piercings, so online ratings are personal reports. Cleveland Clinic notes that needles may hurt less than piercing guns, and the Association of Professional Piercers position on piercing guns warns that guns on cartilage are linked to shattered cartilage and excessive scarring.

Tragus Piercing Healing Time and Aftercare

A tragus piercing takes 6 to 12 months to heal fully, according to Cleveland Clinic, because cartilage has less blood supply than the earlobe. Feeling fine after a few weeks does not mean the channel is healed inside.

A 2026 survey in The Laryngoscope analyzed 9,016 ear piercings from 3,270 adults and found complications reported in 40.2% of cartilage piercings, compared with 25.4% of earlobe piercings. Cartilage piercings such as the tragus therefore need more careful aftercare than a standard lobe piercing (Ziegler et al., The Laryngoscope, 2026).

The abstract is on Wiley Online Library. The APP aftercare guide recommends sterile saline wound wash with 0.9% sodium chloride as the only ingredient, and advises against alcohol, hydrogen peroxide, iodine, antibacterial soaps and ointments. The APP also says rotating jewelry is not necessary and may irritate the piercing.

Tragus healing has one extra challenge: anything that sits in or over the ear canal presses on it. These habits help during healing:

  • Use over ear headphones or the earbud on the other ear until your piercer says the tragus is healed.
  • Take phone calls on the opposite side or on speaker, and wipe your phone screen often.
  • Sleep on the other side, or rest your ear in the opening of a travel pillow, as the APP suggests for cartilage.
  • Keep hair products, makeup and sprays away from the piercing.

Tragus Piercing and Migraines: What the Evidence Says

No clinical evidence shows that a tragus piercing treats migraine. The idea comes from nerve anatomy: NIH StatPearls notes that the tragus is supplied mainly by the great auricular nerve and, to a lesser degree, by the auricular branch of the vagus nerve.

The American Migraine Foundation states that evidence for daith piercing as a migraine treatment is purely anecdotal, with no research supporting the theory, and that reported results are widely believed to be placebo. The Foundation adds that the risks of the piercing outweigh the reward, which is why ear piercings should not replace medical migraine care (American Migraine Foundation, accessed October 2026).

Read the American Migraine Foundation's daith piercing overview. Get a tragus piercing because you love the look, and talk to a doctor about headaches.

Tragus Piercing Risks and When to See a Doctor

The main tragus piercing risks are irritation bumps, infection and, rarely, perichondritis, an infection of the tissue lining the cartilage. Cleveland Clinic explains that the upper ear is cartilage with less blood flow, and that upper ear infections can lead to perichondritis.

A 2025 study in The Laryngoscope reviewed 217 patients treated for auricular perichondritis between 2006 and 2021. Of 120 piercing induced cases, the tragus was the epicenter in 8 (6.7%), while the helix accounted for 105 (87.5%). The tragus carries a real, though smaller, share of serious cartilage infections (Ungar et al., The Laryngoscope, 2025).

Read the study on PubMed Central. See a doctor promptly if the area is swollen, hot, very red or dark, leaking pus, or if you feel feverish or unwell. The NHS guidance on infected piercings advises leaving the jewelry in unless a doctor tells you to remove it. Cleveland Clinic's infected ear piercing overview also lists fever, chills and an embedded earring as reasons to call a provider.

Best Jewelry for a Tragus Piercing

The best starter jewelry for a tragus piercing is a flat back labret stud in an implant grade material, because the flat disc sits smoothly inside the ear. The APP standards for initial piercing jewelry list implant certified titanium, specific surgical steel grades, niobium, solid 14k or higher nickel free gold and platinum, and state that gold plated, gold filled and vermeil jewelry is not acceptable for fresh piercings.

Your piercer may downsize the post once swelling settles. After the tragus is fully healed, a small hoop or a decorative stud changes the look.

Check each product page for materials, and read the hypoallergenic guide for sensitive ears if metals irritate your skin. Then map out your look with our guide to building an earring stack.

Frequently Asked Questions About Tragus Piercings

How painful is a tragus piercing?

A tragus piercing is usually moderately painful and very quick. Cleveland Clinic explains that cartilage is thicker and has more nerves than the fatty earlobe, so cartilage piercings tend to hurt more. Most people describe firm pressure, a sharp pinch lasting seconds and a dull ache for a few days. The sound near the ear canal often surprises people more than the pain. Pain tolerance varies, and no clinical pain scale for piercings exists.

What are the benefits of a tragus piercing?

The main benefits of a tragus piercing are style and visibility. The tragus sits close to the face, so even a tiny stud is easy to see from the front, and it pairs well with a lobe stack. Some people hope it eases migraines, but no clinical evidence supports that, and the American Migraine Foundation calls the evidence for the similar daith piercing purely anecdotal. Choose the tragus for the look, not as a treatment.

What are the downsides of getting a tragus piercing?

The downsides of a tragus piercing are a long healing time, daily friction and infection risk. Cleveland Clinic puts tragus healing at 6 to 12 months, and earbuds, phone calls and sleeping on that side can all irritate it during that window. A 2026 Laryngoscope survey also found complications reported in 40.2% of cartilage piercings versus 25.4% of lobe piercings. Careful aftercare and a skilled piercer lower these risks.

Can I wear earbuds with a tragus piercing?

Avoid in ear earbuds on the pierced side while the tragus heals, which usually takes 6 to 12 months. Earbuds press directly on the jewelry and can carry bacteria into a fresh piercing. Use over ear headphones or an earbud in the other ear instead, and clean any device that touches your ear. Once healed, a flat back stud usually sits flatter under an earbud than a hoop. Ask your piercer before you go back to both earbuds.

How long does a tragus piercing take to heal?

A tragus piercing takes 6 to 12 months to heal fully, according to Cleveland Clinic, compared with about 6 to 8 weeks for an earlobe. Cartilage has less blood supply than soft tissue, so it repairs slowly. Tenderness usually fades within weeks, but the inside of the piercing keeps healing. Wait for your piercer to confirm healing before switching from the starter labret to a hoop or a new stud.
